Abstract

Hospital management is the main key to a successful and efficient health service. The public hospitals are facing an increase of numbers of patients entering the urgent department (UD), increasing the hospitalization waiting time leading the obstruction in the emergency service. Simulation is a method to a study the actual process of hospitalization through designing each steps of the process in Dr.Nélio Mendonça Hospital, a unit located in Madeira Island. Simulation allows to study different scenarios by changing the initial conditions of the system. The aim of this simulation is to study which conditions will affect positively the obstruction of hospitalization queuing time through the emergency department. For this aim, will test 3 scenarios, a growth in bed capacity, a beds capacity of distribution and an increase in the number of UD human resources. The model’s validation and few resources considered constitute limitations to the methodology. The results show that beds capacity distribution scenario improves a higher number of the hospitalization floors, decreasing from 2,5 hours until 14,5 hours when comparing with the actual process. This scenario presents the higher value to the beds utilization rate, approximately 95,4%, translating an increase of 5,6%. However, the last scenario shows a faster response in the UD service presenting a higher number of completed tasks and a higher flow of patients. For these facts, the suggestion is to adapt both changes verified in the two scenarios in order to take advantage of the best results in the two models.
